  • If you would like to have your baby with the Norfolk and Norwich Hospital, please complete their online self referral form.

Asthma

All newly diagnosed asthmatics are assessed in our asthma clinic and established asthmatics are reviewed regularly to make sure their asthma is well controlled.

If you have been advised by the surgery to submit an annual review of your asthma symptoms please use this form. If your symptoms are deteriorating or you are having any concerns please make an appointment with our nurse.

Diabetes

Patients who are treated by diet or tablets are invited to our diabetic clinics twice yearly. Patients on insulin normally attend the hospital.

Family Planning

We provide advice on all methods of contraception, including the fitting of coils, implants, caps and condoms.

If you have been advised by the surgery to submit a contraceptive pill review please see our online services.

Flu Vaccinations

We hold clinics in the autumn of each year, and if you would like to be vaccinated, please request an appointment with our practice nurse. District nurses can vaccinate those who are unable to get to the surgery.

Medicals & Private Reports

If you require an examination or report for driving (e.g. H.G.V.), insurance or employment, we will be pleased to carry these out by special arrangement. Please contact reception to book an appointment. There is normally a fee for this private work to be undertaken.

Minor Surgery

We are able to offer minor surgery to our patients, including removal of moles and cysts, joint injections, cryotherapy and treatment of in-growing toenails. We have a fully-equipped minor surgery room.

Travel Immunisations

The practice is well equipped to offer travel advice. It is very important that if you need travel advice or immunisations you see the nurse well in advance. This should usually be 8-12 weeks before you travel, to plan your vaccination programme.

For more information please see the NHS website.

Please complete our travel risk assessment form, at least 8-12 weeks before you travel.
